Notes: Person in charge brought child to the restaurant; children are not allowed in the kitchen of food service establishments. Lexan containers and sheet pans stacked on shelf for clean kitchenware/utensils had food debris outside and inside containers/pans from stacking with dried/wet food debris; after cleaning, must check for food debris before storage and next use. No food temperature measuring device available to monitor temperatures of food. Food handler wearing false fingernails and not wearing gloves while handling adding pizza toppings to pizzas; if not wearing false nails and/or nail polish, food handlers may wash hands and add toppings to pizzas that will cook over 145 degrees but with false nails and/or painted nails, hands must be washed and gloves put on to put topping on pizzas. Ceiling and diffusers around pizza oven with buildup of dust; must be cleaned frequently. Both acrylic covers missing from fluorescent lights in back storage and extra equipment is stored in the area without covering; equipment must either be stored in boxes or the fluorescent bulbs must be protected in case of breakage.

Notes: No paper towels at hand sink at start of inspection. Knives in bus tub under prep table ready for next use with food debris on blades; after cleaning and sanitizing, knives must be rechecked for cleanliness before storage and next use. Metal plates used to put raw meat and vegetables on for cooking at wok were being rinsed in wok; plates need to be washed, rinsed and sanitized after use to prevent cross contamination. Bags of cooked breaded chicken in sandwich unit on cook line and in walk-in cooler were prepared two days previous per the manager; food prepared and held in a food service establishment for more than 24 hours must be clearly marked to indicate the day by which food must be consumed or discarded. Cooked chicken stocked above top of pan in sandwich unit and unit open continuously during lunch service not allowing foods to be in contact with enough cold air; foods must be kept below top of pans or lid must be closed when not in use.
