Notes: Person in charge not informing employees in a verifiable manner about reporting health and activities as they relate to diseases that are transmissible through food. Both make line coolers in cook area not keeping time and temperature control for safety foods at required 41 degrees or below. One-door merchandiser in dry storage and sandwich until with condensate leaks, both need service. Shelving throughout food preparation and storage area with build-up of grease/food debris. Grease trap located in closet has no lid. Coved baseboard to seal floor/wall juncture is missing throughout prep/cook/washing and dry storage area with large accumulation of crumbs/dirt/debris. Gaps under screen door at back entry to kitchen, under screen doors at side entry to kitchen and under siding on outside of building, where insects/rodents can enter. Many flies inside facility. Evidence of rodent activity behind shelving/equipment in dry storage area.
