Floors still need better cleaning under all cook line and drive thru equipment - accumulation of trash/grease/water/food in these areas. Clean more frequently to prevent these accumulation that can be attraction for pests. Personal food items - jar of garlic, box of spaghetti, bottle of syrup, box of panko breading, bisquick on shelf with rolling pin and flour used for biscuits. Personal items should be stored in a designated area away from food handling and storage areas.

Per owner paperwork is kept at another location - must keep verifiable paperwork for each employee at his location available at his location for review by Health Authority. Pre-made salads in bowls on tray in 2-door merchandiser stacked with bottom of top bowl in direct contact with salad below. Protect salads from any possible contamination by covering salad before stacking. COS: Manager used clean and sanitized trays to cover salads and stacked salads. Cooler door gaskets damaged n both lowboy coolers at Sushi Bar, drink cooler at bar and 2-door reach-in cooler in main kitchen. Keep gaskets in good repair to maintain good seal for maintaining proper cold holding temperatures and to allow for proper/easy cleaning of gaskets. Dumpster missing drain plug. To prevent liquids from leaking out to possibly attract insects/rodents and to prevent possible entry of insects/rodents into dumpster, the drain plug must be replaced. 1/2 lid of dumpster missing. Replace to keep dumpster properly covered. May need to replace dumpster if lid cannot be replaced in the field. Ceiling panels very dusty around diffusers/AC vent in ceiling over prep/cook area and wait/prep area in kitchen. Clean frequently to prevent build-up of dust/debris that can fall onto food/food contact surfaces. Floor needs better cleaning in corners under dry storage shelving, behind water heater and around mop sink and under shelving/corners in walk-in cooler. Accumulation of grease/food/dirt in these areas can be attraction for insects/rodents. Fly glue strips hanging from ceiling over both prep tables in kitchen. These are not approved for use in kitchen since flies may not always stick and can fall into food and onto food contact surfaces. COS: Manager removed fly strips from ceiling in kitchen before inspection was completed. Contact licensed pest control operator for approved fly control that can be used in the kitchen and outside the establishment to prevent fly entry.
