Lisa Marie Berquist Polk, of Cumming, died April 19, 2014.

She was a loving, beautiful daughter, wife, sister, mother, grandmother and aunt.

Her whole life was always dedicated to what she could do for others.

Lisa was a founding member of First Redeemer Church and a graduate of Forsyth Central High School, class of 1988.

She held an associate's degree in business.

Her uncle, Lawrence and Henrietta Palmer of Crown Point, Ind., preceded her in death.

Survivors include her husband, Mike Polk of Cumming; parents, Carl and Joanne Sharp of Cumming, Art and Gail Berquist of Mountain Home, Ark.; brother, Todd and Vanessa Berquist of Mountain Home, Ark.; sisters, Carlene and Ken Jones of Cumming, Bethany Jenkins of Dahlonega; children, Allison Dickerson, Jacob Sharp, Charlie Polk, Molly Polk, Ethan Polk and Emily Polk; grandchildren, Bentley, Ava; niece, Jordan Murdock; nephew, Justin Rothrock; cousins, Michael and Judy Palmer of Crown Point, Ind. and Julie Berquist of Chicago, Ill.; various other relatives also survive.

Service arrangements were incomplete as of press time.

McDonald and Son Funeral Home and Crematory of Cumming is in charge of the arrangements.
